Please have the following documents, in order to be treated at Pain Management Associates of Delaware, faxed or delivered to our office for review by one of our providers before an appointment can be scheduled.

  • Valid Photo ID (Must be a Federal I.D./Passport or State License or I.D card).
  • MRI/CT and Surgical Records or other Radiological Exam from the affected area/s. MRI/CT must be less than two years old.
  • Previous medical records from your primary care and/or current pain doctor detailing your condition.
  • Pharmacy records for the previous 12 months from any and all pharmacies you have used in the last 12 months (your pharmacy can print this out for you).
  • All patients must have a written referral/recommendation from their primary/specialist doctor for chronic pain management either on a prescription (preferred) or the Doctor's office letterhead
  • All current and perspective patients must have and maintain a relationship with a primary care physician
  • Pain Management Associates of Delaware’s new patient hours are by appointment only and follow-up visits are scheduled every 4 weeks.
  • Cash, Check or credit/Debit card; we do not accept insurance; althought you may be able bill your insurance carrier directly for our services as an out-of-network provider. We can walk you through this easy process. HMO insurance plans and all Medicare/Medicaid plans are not eligible for reimbursement.
  • We do not do any types of interventional procedures at Pain Management Associates of Delaware.

Initial Consultation

  • Comprehensive history and physical examination by a pain management staff physician
  • Evaluation of past medical history and records
  • Development of individualized pain management plan
  • Thorough explanation of the appropriate use of pain medications
  • Assessment guidelines for continuum of care
  • Set personal goals
  • Alternative therapies for pain management such as exercise, self-meditation, yoga
  • Office visit: 20-45 minutes

If necessary:

  • Additional screening and diagnostic testing referrals (MRI, etc.)
  • Referrals to non-pharmacological treatment facilities (physical therapy, surgery, psychiatry, etc.)
  • Referral to invasive pain management

Continuum of Care/Follow up visits

  • Physician visit including evaluation of individualized management plan
  • Review of personal goals and quality of life enhancements, including work activities/requirements and social interactions/relationships
  • Evaluation of any new testing or reports
  • Continuation of pharmacological protocol
  • Office visit required every 28 days

If necessary:

  • Referrals for additional testing or evaluation
  • Adjustment of pain management medication protocol
